Some of the information requested on the form is required by the Professional Code and the regulations governing our profession. To avoid any unpleasant surprises, we would appreciate it if you could take a few minutes to check whether the information in your file is up to date and to answer some legal questions, even if you do not wish to be entered on the Roll of the OIIQ for 2008-2009. Remember that if there is any change in your place of residence and/or location of practice at any time during the year, you must inform the Secretary General of the Order within 30 days, by contacting the Registrar’s Office. These data are carefully compiled for statistical analysis, and help the OIIQ formulate positions on various issues, including nursing practice, the nursing shortage, the nursing workforce, basic education, continuing education and other topical matters. Without your help, the Order can’t keep its statistics on the nursing workforce up to date. |
